The federal poverty level determines income eligibility for public health insurance programs in New York State, including the Essential Plan. The Essential Plan aims to bridge the gap for those who may not qualify for Medicaid Managed Care but still need access to quality, low-cost health insurance.

The grants – ranging from $10,000 to $20,000 – are intended to enhance the integration of behavioral health and primary care through innovative strategies. These efforts aim to bridge care gaps for vulnerable populations by promoting comprehensive support. The application is available at fideliscare.org/BH-grants. The deadline to apply is 5 p.m. EDT March 28, 2025.
