Child Health Plus

Child Health Plus (CHPlus) is a New York State-sponsored program that provides quality health insurance for kids under age 19. Children may be eligible, regardless of family income or immigration status.


Child Health Plus benefits include:

  • Medical, dental, and vision coverage for kids up to 19*
  • Checkups, well child visits, and sick visits
  • Hospital and emergency care
  • X-rays and lab tests 
  • Hearing services
  • Occupational, speech, and physical therapy**
  • Prescription and nonprescription medications***

How much does Child Health Plus cost?

Child Health Plus is a free or low-cost plan. You may have a monthly premium based on income and household size. Monthly premiums start at $0 and $15 and go up to a full premium. There are no copays or deductibles. See the chart below for more information on income and eligibility.


When can I enroll in Child Health Plus?

You can enroll in Child Health Plus year-round.

How to Enroll

    • Enroll Online via NY State of Health. Visit NYSOH and click on "Start Enrollment."
    • Complete the Contact Me Form, and a Fidelis Care Representative will be in touch.

    * Always check with your Primary Care Provider to find out if a referral is required for any additional health services you might need.
    ** Some limits may apply.
    *** Child Health Plus covers prescription or non-prescription drugs listed in the formulary as long as they are ordered by a provider within the Fidelis Care network.
