Get your no cost annual checkup
Every year, you are eligible for an annual checkup with your primary care provider (PCP) at no cost. Even if you are healthy, this checkup is important. At the appointment, your doctor will:
Short on time? No problem. Let the doctor come to you with a virtual care option. While certain screenings will not be able to be completed virtually, you can still talk with your doctor about how you have been feeling. This is a good time to talk with them about even small things you don’t think are a problem. Or talk with them about support for stress and anxiety.

The annual checkup is a routine visit with your physician. You can talk about general health questions or issues. You doctor will do a complete review of any ongoing symptoms you may have, and they will do a physical examination. You don’t need to be sick or have symptoms to see your doctor for your annual checkup.
This visit could include all or some of the following: reviewing your health history, checking your height and weight, taking your vital signs, and performing a physical exam.
Fidelis covers both non-emergency and emergency transportationin Rockland County. Non-emergency medical transportation includes: bus, taxi,ambulette, and public transportation.
Transportation must be scheduled inadvance by 3:00 pm the business day before your appointment. If you require anattendant to go with you to your doctor's appointment or if your child is themember of the plan, transportation is also covered for the attendant or parentor guardian. If you have an emergency and need an ambulance, you must call 911.

When you signed up with Fidelis Care, you had the option to pick a doctor from our list of providers to be your PCP.
If you did not choose a PCP within the first 30 days of your membership, one was chosen for you. This doctor has you as a patient and is ready to see you.
Your Fidelis Care Member ID card has your PCP’s name and office phone number. Call your PCP to schedule a visit today! Remember to always carry your Fidelis Care Member ID card with you and show it to the doctor, clinic, or hospital to get the care you need.
