Important Information for People Turning 18 Years Old

When you turn 18, you become a legal adult. Your role will change regarding your health. Others can help with your appointments, but your primary care provider (PCP) will talk to you directly. You will now be responsible for making appointments, checking in to your appointments, filling out forms, and talking with your PCP about your healthcare.

Take Charge of Your Healthcare!

Use this list to get ready for your healthcare as you become an adult. 

    ➪ Have I talked to my PCP about wellness exams? What vaccines and screenings do I need to stay healthy?

    ➪ What is my PCP’s policy about canceling appointments?

    ➪ Is my contact information up to date?

    ➪ Do I have an insurance card? Remember to bring it to appointments.

    ➪ Do I know my own health history and my family’s health history?

    ➪ Do I have a pharmacy for my prescriptions, and do I know what medications I take?

    ➪ Who has access to my medical information? What information about privacy and consent do I need to know about?

    ➪ Will my current PCP still see me after I turn 19? If not, ask if they recommend a PCP.

    ➪ Do I have a care plan? How will this continue or change after I turn 19?

Looking Ahead to Age 19

If you are a member in Child Health Plus or Children's Medicaid, you will no longer be eligible for your current plan after turning 19. Fidelis Care can help you find the plan that's right for you.
