Effective January 1, 2025, Fidelis Care members in Medicaid Managed Care, HealthierLife - HARP, Fidelis Care at Home - MLTC, and Wellcare By Fidelis Care Wellcare Fidelis Dual Plus - Medicaid Advantage Plus plans may be eligible for free access to support services through a Social Care Network.
What is a Social Care Network?
A Social Care Network (SCN) is a group of local community organizations that will provide essential support for housing, nutrition, transportation, and care management. SCNs are designed to help you and your family thrive.
When can I use my SCN?
Starting January 1, 2025, Social Care Networks will be able to connect members with community organizations. Use the below chart to find the Social Care Network in your county. Contact them to find out if you are eligible for services. If you cannot find your SCN, call 1-888-FIDELIS (1-888-343-3547), TTY: 711 for help.
How will the SCN determine my eligibility?
Once connected with your SCN, a Social Care Navigator will ask you a few questions, review your eligibility, explain the available services, and guide you through the next steps to receive the support you need.
