Fidelis Care offers Care Management services to help members who need extra health support. Care Management helps you coordinate care, learn about health conditions, and find helpful resources in your community.

Who qualifies for care management?

Many Fidelis Care members qualify for case management. Case management helps people with ongoing health issues. It also helps people who have mental health concerns or people who have a substance use disorder. To receive more information, please call Fidelis Care Clinical Services at 1-800-247-1441.

Are you a HealthierLife | Health and Recovery Plan (HARP) member?
All HealthierLife members are automatically eligible for care management from our dedicated HARP team. If you are a HealthierLife member, call 1-888-FIDELIS (1-888-343-3547), ext. 16072 for help.

Are you a Wellcare By Fidelis Care member?
Wellcare By Fidelis Care also offers case management to members by phone. Case managers can provide support and assistance in identifying alternatives and resources when benefits have been exhausted. Call Fidelis Care Clinical Services at 1-800-247-1441 for help.
