Documents Needed to Enroll


Congratulations!  You've submitted an inquiry and are one step closer to finding the optimal insurance plan for you.

While you're waiting on a call from one our of Certified Application Counselors, here are a few items/documents you can gather to ensure this eligibility/determination process is quick and easy on your busy schedule: 

  1. Social Security numbers (or document numbers for legal immigrants who need health insurance)
  2. Birth dates
  3. Employer and income information for everyone in your family
  4. Policy numbers for any current health insurance
  5. Information about any job related health insurance available to your family

We keep all of the information you provide private, as required by law.
