In an effort to streamline provider termination submissions to Fidelis Care, we have created a new electronic Termination Request Form for providers to use.  Please use this form to submit any requests to terminate a provider from a TIN. In order to expedite the changes requested, all fields on the form are required. If not complete, the request will be delayed. Once completed, the form can be submitted electronically to SMProviderAttestationUpdates@fideliscare.org. In the subject line of the email, please enter Termination Request for "Your Provider/Group Name" in "Your County" (i.e. Termination Request for ABC Healthcare Group in Ontario County). Wellcare Fidelis Dual Plus is a health plan for individuals who have Medicare and Medicaid coverage. Enrollment in the program includes Medicare, Medicaid and long-term-care benefits, allowing for better coordination of care for our members.   Our Medicaid Advantage Plus (MAP) plan is part of the WellCare by Fidelis Medicare plan suite – and thus, if providers are in-network for Wellcare by Fidelis Medicare, then they are also in-network for our Medicaid Advantage Plus (MAP) plan.   Members enrolled in Wellcare Fidelis Dual Plus plans receive individualized support and flexible care planning to help them live in their own homes and communities for as long as possible. In an effort to improve the quality of care for our members, Fidelis Care will be requesting Essential Plan members select a Primary Care Provider (PCP) starting in 2024.  The initial outreach will begin with members renewing their coverage as of March 1, 2024.  Members will be prompted by our Renewal Team to select a PCP when renewing their coverage.  If a member does not designate a PCP, Fidelis Care will assign a PCP to the member.   Providers are encouraged to share with their patients the benefits of having a PCP to ensure health care needs are addressed during annual physicals

Fidelis Care is pleased to announce that, effective January 1, 2024, Express Scripts® began processing pharmacy claims for our plan members. Express Scripts is a pharmacy benefit management (PBM) company serving more than 100 million Americans. Express Scripts Pharmacy delivers specialized care that puts patients first through a smarter approach to pharmacy services. Providers can direct members to call the Member Services phone number listed on their ID card should they have questions about this change. Alternatively, Fidelis Care has created an Express Scripts FAQ for providers to reference in order to assist members. The FAQ also includes information on mail order
