Fidelis Care at Home is a managed long-term care program for residents of Albany, Allegany, Bronx, Broome, Cattaraugus, Cayuga, Chautauqua, Chenango, Columbia, Cortland, Delaware, Dutchess, Erie, Essex, Fulton, Genesee, Greene, Hamilton, Herkimer, Kings, Livingston, Monroe, Montgomery, Nassau, Niagara, New York, Oneida, Onondaga, Ontario, Orleans, Orange, Oswego, Putnam, Queens, Rensselaer, Richmond, Rockland, Schenectady, Schoharie, Steuben, Suffolk, Sullivan, Tioga, Tompkins, Ulster, Warren, Washington, Wayne, Westchester, or Wyoming counties who need long-term care services and have Medicaid. Members have their own Care Manager, receive individual attention, and have the support and resources they need to help them live independently in their own homes and communities.
