Fidelis Care Here for Your Health Maternal Health Grant Program

NOW ACCEPTING APPLICATIONS

Fidelis Care is awarding Here for Your Health maternal health grants to healthcare providers and community-based organizations in New York State to help support programs that advance innovative solutions to improve postpartum visit adherence and expand postpartum depression screening.

If your office or organization runs a program that helps supports these types of initiatives, particularly for high-need, low-income women and their families, then Fidelis Care encourages you to apply for the grant.

Grants up to $20,000 will be awarded. To be eligible, providers or community organizations must serve New York State. Applications will be accepted until 5 PM on September 21, 2026. Applicants must must operate a program focused on postpartum visit adherence and/or postpartum depression screening and answer all of the questions on the form to be considered.

Please note: Some reporting is required. Awards will be announced in October.
