Fidelis Care Here for Your Health Behavioral Health Grant Program

Fidelis Care encourages providers and organizations to apply
for funding to promote initiatives that prioritize programs that improve access, engagement, and continuity of care for underserved and historically marginalized populations.

Grants of up to $20,000 will be awarded. To be eligible, providers or community organizations must serve New York State. The deadline to apply is no later than 5 p.m. Wednesday, April 1, 2026.

Fidelis Care plans to announce the recipients of its 2026 Here for Your Health behavioral health grants in May in observance of Mental Health Awareness Month.

• Complete the online Fidelis Care Behavioral Health Program Grant Application Form and click submit.
• To assist with preparing your application, a reference is available outlining the application questions.
