Introducing the Social Care Networks (SCN) Program under the NY 1115 Waiver Effective January 1, 2025

As part of New York’s Medicaid Redesign Team (MRT) initiative, the NY 1115 Waiver introduced the Social Care Networks (SCN) Program, a transformative model focused on integrating social care into healthcare delivery. There is growing recognition that fully achieving health for all requires a focus not only on physical and behavioral health, but also on Health-Related Social Needs (HRSN). The SCN program encourages partnerships across health and social services to address Medicaid members' diverse needs, such as food insecurity, housing instability, and lack of transportation. Many of these HRSN services may become reimbursable to a member under the provision of this waiver.  

How the SCN Program Works

Starting January 1, 2025, eligible Medicaid, MAP, MLTC, and HARP Members will have access a wide range of free services through the local Social Care Network (SCN), which connects members to organizations in their community. These services provide essential support for housing, nutrition, transportation, and care management to help members, and their families thrive.

A dedicated Social Care Navigator, through the SCN, will meet with members to assess eligibility for services that support their health and well-being. After a simple assessment, the Navigator can connect members to the right services tailored to their needs. Depending on the situation, members may qualify for one or more of these resources:

Housing and Utilities Support

  • Home modifications for safety and accessibility, such as ramps, handrails, and grab bars.
  • Repairs to prevent mold and pest issues, including sealing cracks and fixing leaks.
  • Access to essential appliances like air conditioners, heaters, and dehumidifiers for a healthier home environment.
  • Assistance in finding and applying for stable housing within the community.

Nutrition Support

  • One-on-one guidance from a nutrition expert to help you make healthy food choices for your specific needs and goals.
  • Up to six months of home-delivered meals, fresh produce, and groceries tailored to your health requirements.
  • Essential cooking supplies (pots, pans, microwave, refrigerator, utensils) to support meal preparation at home.

Transportation Services

  • Reliable transportation to essential destinations approved by the SCN, including job interviews, parenting classes, housing court, local farmers’ markets, and government offices.

Care Management Services

  • Comprehensive support with job placement, training, financial management, and applications for public benefits.
  • Connection to childcare, counseling, crisis intervention, health home programs, and other critical services.

Beginning January 1, 2025 members will be able to connect with their regional SCN to determine eligibility for services.  The regional SCN’s contact information will be available on fideliscare.org on January 1, 2025.
