Fidelis Care New York is a health plan with a Medicare contract. All of our Medicare Advantage plans are Point-of-Service plans. This means you have the option of seeing a provider who is not in the Fidelis Care network. Higher copayments will apply. See Sections 1 and 2 of our Evidence of Coverage documents for more details.
You must continue to pay your Part B premium in addition to any Fidelis monthly plan premium. The monthly Fidelis plan premium covers medical services and prescription drug benefits for those plans that offer prescription benefits. Your premium will be lower if you receive extra help from Medicare.
Fidelis Care offers a Flex benefit that covers items and services that Medicare does not normally cover, such as eyewear, dental services, over-the-counter items and many other services. To use the Flex benefit, you must pay for the item or service out-of-pocket, obtain a receipt, and submit the receipt along with the Flex Reimbursement form to us. We will give you a check up to the Flex amount available under the plan you chose. The following plans include a Flex benefit: Fidelis Medicare Advantage without Prescription Drugs, Fidelis Medicare Advantage Flex, Fidelis Medicare Advantage, Fidelis Dual Advantage and Fidelis Dual Advantage Flex.
