Individuals who are 18 years of age and older may apply for enrollment in Fidelis Care at Home if they:

• Are eligible for Medicaid
• Reside in Orange or Rockland County
• Are eligible for nursing home level of care
• Require the long term care services offered by Fidelis Care at Home for at least 120 days or more from the date of enrollment
• Are capable, as of the time of enrollment, of remaining in their home and community without jeopardy to their health and safety.
