Renewing Your Health Coverage with Fidelis Care is Easier Than You Think!

Every year, members enrolled in Child Health Plus and Medicaid Managed Care Programs in New York State must renew their health care coverage. This process is called recertification. Fidelis Care is here to help you every step of the way in completing the forms to keep your health care coverage. Here's how to get started: 

  • Call 1-866-435-9521 and a Fidelis Care representative can:
    • assist you in completing the forms over the telephone
    • schedule an appointment for you to meet with a representative
    • provide a list of our locations where we can meet and assist you
  • Visit us at one of our locations for assistance
  • Complete the forms using blue or black ink and mail them back

If You Have Medicaid Managed Care Coverage:

About 30-60 days before your coverage is due to end, you will receive a recertification package in the mail that will contain a renewal application from your local Department of Social Services (LDSS) or the Human Resources Administration (HRA), if you live in New York City.

To keep your coverage:

  • Review the information carefully and make any necessary changes
  • Submit the required proofs 
  • Sign and date the form
  • Mail the form as soon as possible in the envelope provided.
    If the form is not received by the date due, your coverage will end.

When the Department of Social Services or HRA receives your renewal form, a review will be done to determine if your eligibility for Medicaid Managed Care has changed. If your eligibility is confirmed by the Department of Social Services, Fidelis Care will continue as your health plan.

If You Have Family Health Plus

As of January 1, 2014, New York State is not accepting new applications for Family Health Plus.

When it is time for you to renew, you will have new options. To understand what you're eligible for, follow these simple steps:

1. You will receive a renewal (or recertification) form approximately 30-90 days before your coverage is due to end. You will need to complete and submit this form.
2. After submitting your renewal form, you will receive notification from your local Department of Social Services (LDSS) or the Human Resources Administration (HRA) with the following information:
  • You are eligible for expanded Medicaid coverage. No further action required.

OR

  • You are not eligible for expanded Medicaid coverage. Please visit the Fidelis Affordable Care Advisor to explore your options.

To maximize your subsidy potential, you will need to choose the Silver Plan through NY State of Health. If you have any questions, call 1-866-435-9521 and a Fidelis Care representative can assist you.

 

If You Have Child Health Plus:

Ninety days before your child’s coverage is due to end, Fidelis Care will mail you a recertification package with a renewal form.

To keep your coverage:

  • Complete the renewal form 
  • Submit the required proofs 
  • Sign and date the form 
  • Mail the form as soon as possible in the envelope provided.
    If the form is not received by the date due, your coverage will end.

Upon receipt of your renewal form, it will be reviewed to determine if your child's eligibility status has changed.
